Active & Creative Kids Vouchers

Shishkova Ballet Studio is a registered provider for both the NSW Active Kids vouchers and NSW Creative Kids vouchers for school-aged children. 

Parents need to apply for the vouchers via Service NSW with the eligibility criteria set out by the Office of Sports NSW.

Once you have your voucher, please complete the below form (one form per voucher) and enter your details as it appears on your Service NSW voucher.

The voucher will then be deducted from your account. The vouchers can only be used for class term fees and are only valid for classes in the same calendar year applied.
